Changing Gender on Your NHS Medical Record

We are committed to supporting transgender and non-binary patients with respect, dignity, and confidentiality.

If you wish to change the gender recorded on your NHS medical record, there are two options available. You can choose the option that best suits your needs.

Your Options

 

1. Keep Your Current NHS Number

If you choose this option:

  • Your NHS number will stay the same
  • Your historical records will still show your previous identity
  • All future records will reflect your updated identity

 

2. Request a New NHS Number

If you choose this option:

    • You will be given a new NHS number
    • Your previous NHS number will be deactivated
    • A new medical record will be created in your updated identity
    • Information recorded before your transition will be removed (redacted) from your new record
    • You will need to inform any current care providers (e.g. hospital specialists or clinics) of your new NHS number

 

We will ask you to confirm your choice (via text, letter, or email)

Screening and Your Health

It is important to continue attending routine screening appointments based on your sex assigned at birth. This helps ensure you receive the most appropriate care.

For example, you may still need:

  • Cervical screening
  • Breast screening
  • Prostate checks
